Progress of labour is determined by 4 hourly vaginal examinations to assess
cervical dilatation and abdominal examination to assess the descent of the
head. The cervical dilatation is the best parameter to determine the progress of
labour, as it is recorded graphically in the partogram and the normogram, alert
(2 hours to the right) and action lines (4 hours to the right) are available for
comparison. However, the descent of the head should progress together with the
cervical dilatation. If labour is progressing normally, the head should be fully
engaged and should be at the level of the ischial spines, when the second stage is
reached. Uterine contractions are essential for the progress of labour and are
recorded half hourly. However, the frequency of uterine contractions is not a
parameter of progress of labour. When labour is obstructed, progress can be
arrested in the presence of strong uterine contractions. Cervical dilatation is
assessed 2 hourly only in the presence of a complication, such as delayed progress.
The dose of oxytocin is adjusted to obtain adequate contractions, but is not a parameter of progress of labour.
